Background & Objective: Globally 3 million deaths, 5.3% of all deaths annually, 5.1% of the global burden of disease and injury measured by Disability-adjusted life years (DALYs) and causal relationship with more than 200 diseases and 35% deaths among 20-39 years are evidence of massive destruction incurred by Alcohol and illicit liquor. The scenario becomes graver in LMICs like Pakistan having the inverse demographic pyramid.

Material & Method: This was a cross-sectional, exploratory study in big cities like Karachi, Sukkar, Islamabad, Lahore, and Faisalabad--Pakistan. The analysis was done by using in-depth interviews with EMS & Law enforcement personal, substance users, and paddlers. The qualitative inquiry (FGDs and interviews) explored the determinants of substance abuse and the reasons for fatalities in urban and rural communities.

Result & Discussion:

(1) During festivals, marriage ceremonies the consumption was 80% higher (2) There is a direct relationship between poverty and corruption of law enforcement with availability and accessibility of cheaper liquor. The availability is significant in minority communities where economic disadvantages are prevalent. Their criminal syndicate mushroom, political unrest situations are common and rules are ignored (3)The impact of illicit liquor is massive. (4) Poverty and stress increase the demand, especially in young adults.

Learning objectives

An integrated approach is needed to halt the lucrative trade, availability of illicit liquor, and optimize probation with improved rules implementation. This will ensure safety and control of menace and save scores from untimely miserable deaths.
